Requirements and Information for Inpatients Check-out Process

1. The inpatient shall forward the check-out process upon attending doctor’s instruction. For self-motivated check-out, the inpatient shall fill in the Voluntary Check-out Voucher Document.
2. The secretary of the ward will provide the inpatient a Report of Medical Treatment Cost and Check-out Notice Document for the inpatient to pay his/her medical treatment charges.
3. The inpatient then goes to the charge window to pay his/her medical treatment charges. Upon completion of the payment process and receipt of leaving pass, the inpatient returns the leaving pass to the Nursing Station and is ready to leave the hospital.
