Requirements and Information for Inpatients-Application for Relevant Materials

(I.E., Medical Treatment Records and Certificate of Diagnosis)

Application Time

Monday to Friday: 8:00~11:30 and 13:30~16:00

Application Location

On the 1st floor of Registration Room in the Medical Service Building and Inpatients Registration Office Counter.

Required Status for Application

In according with Article 74 of the Medical Law, the patient or his/her legal representative is entitled to apply for the transcripts of Medical Treatment Records, including

  • Patient himself/herself
  • Legal representative: legal spouse
  • Relatives: the priority sequence for application is as follows (1) Spouse, (2) Parents, (3) Adult children, (4) Adult brother/sister, (5) Grand parents, (6) Other adult relative(s)
  • Need to submit the Authorized Paper clearly stated the authorized will and scope by the patient or his/her legal representative.

Required Identification Documents

  • The applicant is the patient himself/herself (National Identification (ID) Card), other or legal representative (with Authorized Paper clearly stated the authorized will and scope by the patient or his/her legal representative)?
  • The inpatient (carrying National Identification (ID) Card) can forward the application himself/herself or his/her family member carrying National Identification (ID) card can forward the application for the inpatient (the assignee need to carry his/her own National Identification (ID) Card, and assignor's National Identification (ID) Card and Authorization Paper)

Reproduction Process of Medical Treatment Records

  • The reproduction of the entire book of Medical Treatment Records (including Inpatient Residence Treatment Curse Records, Nursing Records, Medical Order #1 for Outpatient Visit): registration > fill in application form > verify identification (Treatment Room, Registration Room, the Nursing Station) > check and transfer Medical Treatment Records (Medical Records Room) > reproduction of relevant Medical Treatment Records (Medical Treatment Records Room) > doctor's verification (doctor) > payment (Registration Room) > chopping (printed words that mean identical to the genuine transcript) > signing and receiving (Note: the inpatient has to display his/her own identification certificate; for heavy illness inpatient who is unable to display his/her identification certificate, his/her spouse or relative need to apply for him/her)
  • The reproduction process of Medical History Abstract or Examination Reports/Materials for check-out inpatient: registration > fill in application form > verify identification (Treatment Room, Registration Room, the Nursing Room) > check and transfer Medical Treatment Records (Medical Records Room) > reproduction of relevant Medical Treatment Records (Medical Treatment Records Room) > doctor's verification (doctor) > payment (Registration Room, Inpatients Registration Office Counter) > chopping (printed words that mean identical to the genuine transcript) > signing and receiving (Note: the inpatient needs to display his/her own identification certificate; if no relative relationship, the assignee need to display the Authorization Paper clearly stated the will of assignor) (Doctor's verification: the original and attending doctor shall be the first to verify; if the attending doctor is not available, the director of the same specialist division shall then verify; for emergency care, the second line doctor shall verify.)

Explanation of Reproduction

  • According to Official Document of Ref. No. Wei Shu Yi 0930217501 issued by the Department of Health on September 30, 2004, the basic cost for reproduction of Medical Treatment Records shall be a maximum of NT$200, with an addition of NT$5 for each additional paper sheet.
  • According to Article 74 of the Medical Law, the hospital shall provide the reproduced transcript(s) of Medical Treatment Records and various reports of examination materials at patient's own cost at the request or consent of the patient, or his/her legal representative, spouse, relative or relative person.
  • According to Official Document of Ref. No. Wei Shu Yi 0930217501 issued by the Department of Health on September 30, 2004, to reduce the labor effort and cost of transportation of the applicant, the hospital can also mail by post mail the applied document(s) at patient's own cost.
  • The reproduction of relevant Reports of Examination and English version transcript(s) of Medical Treatment Records requires 1 to 3 business days, the reproduction of the entire/part of the Medical Treatment Records requires 3 to 14 business days. Basically, Chinese-version Medical Treatment Records requires 14 business days.
  • Requirements on application for receiving the applied documents: (1) When the patient applies, he/she has to carry identification certificate; for the applicant who is not the patient himself/herself, the applicant has to display patient's Authorization Paper, together with patient's National Identification (ID) Card and applicant's National Identification (ID) Card. The charge shall be paid based on the public notice standards displayed on the Front Counter. (2) Mail by post mail: Please prepare or request the envelop from the Front Counter. The envelope needs to clearly write down the name, address, and telephone number of receiver. The applied Medical Treatment Records will be mailed at patient's own cost upon completion of the reproduction process.

01 How to apply "General Diagnosis Certificate"?
  (1) Outpatient registration > doctor issues > Payment at the Front Counter > Receive certificate(s) with hospital's official chopping


(2) Inpatient can request the doctor to issue, and then he/she or family member carrying National Identification (ID) Card to receive at the Inpatients Registration Office Counter after payment is made 3. Apply for foreign nursing labor(Illness and Disabled Diagnosis Certificate)from this office, and this office will forward the application to the Long Term Nursing Center of Taoyuan County for 14 days in processing.

How to apply for "Medical Treatment Records Abstract?and "Photocopied Report of Examination?
  (1) After completion of registration, the inpatient receives outpatient diagnosis by doctor, and then forwards his/her application to the doctor or to the window of the Registration Room window (after displaying National Identification (ID) Card for verification), and then continues the process at the Registration Room.


(2) In patient or his/her family member carrying National Identification (ID) Card can forward his/her application to the Nursing Station.


(3) Required process days upon application
(a) For Reports of Examination, Medical Treatment Records Abstract of English version: 2 business days
(b) Reproduction of the entire/part Medical Treatment Records: 10 business days
(c) Medical Treatment Records Abstract of Chinese version: 14 business days
03 How to apply for a "Death Certificate?
  Dead patients at long term nursing medical institute, homecare and this hospital upon check-out leaving, his/her spouse or immediate family members carrying genuine National Identification (ID) Card forwards the application to the Nursing Station of the ward (assignee of legal application status needs to carry his/her own genuine National Identification (ID) Card and legal Authorization Paper, genuine National Identification (ID) Card of the assignor)

How to apply "Date of Birth Certificate of Chinese version?and "Date of Birth Certificate of English version?
  (1) Date of Birth Certificate of Chinese version: Parents of newly born baby needs to carry genuine National Identification (ID) Card or Household Registration Certificate to apply at the Nursing Station of the ward


(2) Date of Birth Certificate of English version: The applicant himself/herself or family member carrying passport with Date of Birth Certificate of Chinese version and genuine National Identification (ID) Card or Household Registration Certificate to apply (assignee needs to carry genuine NATIONAL IDENTIFICATION (ID) Card and Authorization Paper) at the Hospitalization Room. The applied certificate cannot be issued on the same day when applying.

How to re-apply "Receipt Certificate? What is the difference between issuance of lost receipt certificate and addition of receipt certificate?
  Apply for issuance of lost "Receipt Certificate?at the Inpatients Registration Office Counter or Registration Window at a cost of NT$20 for service charge. Apply for additional copies of "Receipt Certificate?at the Inpatients Registration Office Counter or Registration Room using original Receipt Certificate at a cost of NT$20 for each copy service.
06 How to apply the disabled benefits with labor or agriculture insurance?
  Please request blank application form on the disabled benefits with labor or agriculture insurance from insurance application unit, and then ask the attending doctor to issue such form. After payment is made and the application form is chopped, the application form shall be forwarded to the insurance application unit.

How long it will take to apply for new certificate one before the existing Serious Injury and Illness Certificate Card expires?
  Apply for new certificate card one month before the existing certificate card expires. Also, the application has to be filed within 30 days upon the application for serious injury and illness certificate is approved by the attending doctor.
08 How to apply for issuance of lost or addition of Diagnosis Certificate?
  The patient or his/her family member carries genuine NATIONAL IDENTIFICATION (ID) Card (assignee needs to carry his/her own genuine NATIONAL IDENTIFICATION (ID) Card and the Authorization Paper and NATIONAL IDENTIFICATION (ID) Card of the assignor) to the Registration Room Window or the Inpatients Registration Office Counter to apply.
